CORONAL PRE-ENDODONTIC RESTORATION

Case 3. - Female, 59 years old, with a very extensive loss of structure in her first right mandibular molar (4.6).- Decayed tissues were removed, keeping all the structures that were possible, even undermined enamel that had lost its supporting partner, sound dentin.
Pulp tissues are partially eliminated and dentin pins are threaded in sound dentin, using relative isolation. (fig.14).

Adhesive dentistry and dentin pins were used in this case to reinforce the remaining walls, as well as to build up the ones that were lost, in order to accomplish absolute isolation of the operatory field (figs 15 and 16 a-b).

Gutta-percha point in figure 14 is positioned in canal so as to avoid adhesive and composite resin flowing into distal canal while walls were being rebuilt.
After the endodontic filling was accomplished, a core with passive radicular posts and adhesive dentistry was done, as was previously described in case 2.
Then, the coronal portion was prepared in order to receive a metallic 7/8 crown, with its margins adjusted in sound tooth structure (fig.17 a-b).
